TEAM REGISTRATION Online Instructions 1. Submit Part A: Registration online. 2. Download and complete Part B: Team Roaster and Part C: Waiver from each team member. Write a check of appropriate amount to "Hong Kong Dragon Boat Festival in New York" submit along with Part B: Team Roaster and a signed copy of Part C: Waiver from each team member. Mail Check, Part B and Part C to 141-07 20th Avenue, Suite 406, Whitestone, NY 11357-3097 (indicate your Team Name on the check). Standard Mail Instructions 1. Download the complete package. 2. Fill out and mail Part A, B and C 3. Write a check of appropriate amount to "Hong Kong Dragon Boat Festival in New York", submit along with Part B: Team Roaster and a signed copy of Part C: Waiver from each team member.
